Artist Release Form

The undersigned hereby releases and agrees to hold harmless The University of Toledo, The University of Toledo Alumni Association and its sponsors from any damage or loss to the undersigned's property or any personal injury which he/she or helpers may sustain while setting up on Saturday, July 27 or participating in ART ON THE MALL on July 28, 2024.

Further, I agree to abide by the rules, policies and guidelines developed by the ART ON THE MALL COMMITTEE, The University of Toledo and The University of Toledo Trustees and understand that failure to follow these regulations can mean expulsion from this year's fair and future events.

ART ON THE MALL does not carry insurance to cover your personal property. As an independent contractor, you are advised to obtain your own insurance.
